• Passport Replacement Procedure:

  Requirements :

1. Completed application form (Perdim 14); Download
2. An original passport and 1 copy of front page's passport;
3. Original A.R.C ( Alien Resident Certificate ) and 1 copy of A.R.C;
4. 4 color photos with white background + include photo file in a CD, size 4 x 6 cm (2 inch);
5. Finger print and Signature on the application form.

• Passport Extension Procedure:
  Requirements :

1. Completed application form (Perdim 14);
2. An original passport and 1 copy of front page's passport;
3. Original A.R.C ( Alien Resident Certificate ) and 1 copy of A.R.C;
4. 4 color photos with white background, size 4 x 6 cm (2 inch);
5. Finger print and Signature on the application form.

• Replacement Procedure of Lost Passport:
  Requirements :

1. Completed application form (Perdim 14);
2. 1 copy of front page's passport;
3. Original A.R.C ( Alien Resident Certificate ) and 1 copy of A.R.C;
4. 4 color photos with white background + include photo file in a CD, size 4 x 6 cm (2 inch);
5. Lost passport evidence letter from local imigration office;
6. 1 day newspaper advertisement of losing passport announcement;
7. Finger print and Signature on the application form.

• New Child Passport Application Procedure:
  Requirements:

1. Completed application form (Perdim 14);
2. Child Birth Certificate (English version) with Taiwan judicial approved;
3. Marriage Certificate;
4. Birth certificate of Father / Mother;
5. Indonesian ID number (KTP) of Father / Mother;
6. ARC of Father / Mother;
7. Family resident card ( for Taiwanese mother citizen ).
